Job Opportunity – Telephonic Home Care Patient Access Liaisons

Care Liaisons are the primary point of contact for patients that are enrolled in Vree Health programs. Care Liaisons support patients for the month after they are discharged from the hospital by providing daily well checks over the phone by using coaching techniques, such as Motivational Interviewing, Positive Psychology and Goal Setting. Care Liaisons also collect medical information, assist patients and family members by monitoring health status, coordinate doctor appointments, work with pharmacies to ensure timely refill of prescriptions, and alert the patient's health care team concerning condition changes. 
Our ideal candidate is someone who is compassionate, empathetic, and maintains a positive attitude. You must have excellent communication skills and the ability to listen and assist patients who may be experiencing a challenging or difficult time. You must be comfortable gathering medical information and talking to patients who have just been released from the hospital.
This is not a clinical position--experience as a hospital patient advocate or admissions coordinator would be ideal.
We have full-time and part-time positions available, with multiple shift start times to provide patient support from 7am to 7pm daily.
