SOCIAL WORK position in a 200 bed LTC/SAR facility. Looking for someone who
is enthusiastic about working with the geriatric population.
Responsibilities include conducting IDT family care plan meetings, MDS
assessments, coordinate discharge planning as well as 1:1 interaction with
residents and families. LSW and one year experience in a long term care
